Community Health

Medfleet’s PatientCare Community Health Division was launched to build on its legacy of being a trusted, reliable, compassionate provider for the people of Central and Western Florida. This service expands the scope of healthcare to support insurance companies, hospitals, primary care physicians and other healthcare providers in assuring quality patientcare is delivered effectively and efficiently in the areas we serve.

Through PatientCare Community Health, we are able to help reduce or eliminate patient’s needs for additional visits to healthcare providers, and achieve more positive outcomes for longer-term health. We assist in alleviating providers’ high patient loads and help insurance providers fulfill their commitments to plan members in a way that prioritizes good health while controlling escalating costs and keep the patient in their home.

The services are currently being delivered in Pasco, Polk, Hillsborough, Hernando, Hardee, Highlands, Osceola, Seminole, Orange, and Citrus Counties.

PatientCare Community Health Team

Frequently patients leave the hospital and need some level of follow up care but that care often does not need to happen in a hospital. It can be taken care of in the home by highly trained and qualified paramedics or registered nurses who have phone and video access to physicians if needed.

  1. Delivers in-home patient care by a certified NAEMT Community Paramedic with a Community Health Worker certification
  2. Perform visual inspections of the home’s exterior to determine if it is in disrepair, if the yard is maintained and generally safe for the patient
  3. Assesses interior conditions for unsafe food or other conditions
  4. Determines if medications are in sufficient supply, stored appropriately and being administered correctly
  5. Determines if patients are attending telehealth visits as needed and assisting with appointments
  6. Conduct clinical assessments including ear, nose, and throat, blood pressure, heart and respiratory rate, weight checks, etc.

We provide ongoing certification and other training to our employees including a leadership development program designed specifically for our team.
